There is no legal requirement to take your spouse's name. However, If you do decide to take your husband's name after your wedding, you will need to obtain official certified copies of your Marriage License (also called a Marriage Certificate) from the County Clerk's office. The first place you should start is at the Social Security office. You should file an SS-5 form (a new application form showing change of name and/or marital status) within the fiscal year for tax purposes, even if you are keeping your own name, along with an official copy of your marriage license. Check with Social Security first... ask if you walk it in, will they just look at it and return it to you.

Investigate your state's policy on changing your name after marriage. If either spouse is adding the other to his/her insurance policy, that usually is done right away. INS records are also commonly dealt with rather promptly. You will need an official certified copy of your Marriage License in order to change your name at the Motor Vehicle Bureau, your bank, health insurance and sometimes even your credit cards.
